As your Patient Centered Medical Home (PCMH), chronic disease management is important to us! Our Asthma Care Coordinator, MoRhonda, will work with patients who have Asthma to help manage their care and follow-up visits. She works closely with our patients, asthma nurses, and physicians to help organize our Asthma Care Clinics:

  • Asthma Care Clinics are for children ages 7 years and up
  • Focus on asthma prevention by updating your child’s Asthma Action Plan
  • Review medication management
  • Provide education about asthma
  • Perform Spirometry, which is recommended annually for patients with asthma and can be a good tool for assessing your child’s asthma severity
  • Asthma clinic patients will also get their annual flu shot, if available
